Appointment Waiting Time
You are welcome to call the office shortly before your scheduled appointment to find out if the doctor is running on time. If at that time, we have any reason to believe that the doctor will be running behind, we will give you an approximate time to arrive that will eliminate as much waiting room time as possible. Please understand that there will be unforeseen problems that arise and that this will not always be effective.
Emergencies
In case of an emergency, please go directly to the Emergency Room of the nearest hospital. The Emergency Room staff will contact our office or the physician on call if the need arises. You have the right to request further care and treatment from Chippewa Valley Orthopedics and Sports Medicine.
Insurance
As a service to our patients, we will file standardized insurance claim forms to all insurance carriers for primary, secondary, and tertiary carriers. It is the patient’s responsibility to determine whether our services will be covered by their plan and obtain any necessary referrals prior to their scheduled appointment. The patient remains responsible for any charges remaining after insurance payment and for any non-covered services. You will begin receiving a monthly bill once we have received payment from your insurance.
Patient Information Forms
New patients or those patients being seen for a new condition, will be asked to complete a medical history form upon their arrival for a scheduled appointment. Medical Release Form - complete so we can request your past medical records be sent to us if needed, or if you would like your medical records with us sent elsewhere.
Notice of Privacy Practices - this document reviews the privacy practices of Chippewa Valley Orthopedics and Sports Medicine as well as informs you of your privacy rights with respect to your personal health information.
